West Devon CVS has an exciting opportunity for a compassionate and enthusiastic individual who is eager to help others as Co-ordinator for the Core20+ programme.

The programme is one of a number of funded initiatives that support focused action across Integrated Care Systems (ICS) to reduce healthcare inequalities across five priority clinical themes and contribute to wider steps on health inequalities including upstream social determinants.  https://www.england.nhs.uk/about/equality/equality-hub/core20plus5/

The role of the co-ordinator is to recruit, co-ordinate and support a team of 20 volunteer community connectors who will be based within their local communities across the North Dartmoor PCN area. To help understand and contribute to the reduction of health inequalities in the national and local priority areas and to report to the Project Steering Group via West Devon CVS.

The volunteer community connectors/groups will be embedded in and engage with local communities acting as a ‘bridging’ role to ensure understanding of current barriers experienced by users and how this can be mitigated.

To be considered for the position of Co-ordinator, you must have good IT skills, the capacity to work both independently and as part of a small team. The ability to communicate compassionately and respectfully and be creative about how we can capture the voices of those people who are affected by health inequalilties using all mediums e.g. case studies, blogs,vlogs or personal diaries.

Additionally, you must be able to promote client equality and diversity at all times and attend mandatory training courses when necessary.  

The post is part time at 20 hours per week and will be largely based from the Community Hospital in Okehampton. The majority of the hours will be daytime but some evening work may be necessary. There is an expectation that you will be able to travel around the PCN area locality.  Salary: £12.57 per hour
